In the month of January 2022 I visited our patients and their home carers in 5 communes, getting to know their medical and non-medical needs. In 3 cases, after consulting our hospice physician and writing down a note about the visite, a decision was made that these patients did not require our home hospice care yet.

However, after visiting a lady from the village of Wiejki, following the phone notification, a decision was made to take her under our care.

In order to meet other needs, in cooperation with the network, I have taken the following actions:

1. Social welfare home in Jałówka employee inserted the glass and sealed the window for one of our patients.
2. I helped in the submission of 2 applications to the County Family Assistance Center for funding the purchase of access ramps.
3. I helped the patient’s daughter, the home caretaker, in submitting an application for a disability group for her father.
4. I brought a leg rotor, because the patient was not able to come to the hospice rental by himself.
5. During the visit of the Ombudsman and his team, I participated in talks and visits to our patients.

As part of educational activities, I prepared meetings with a home hospice nutritionist for the home carers and members of the Rural Housewives’ Association, Associations and employees of the Communal Social Welfare Center and Social Assistance Center in five municipalities. The topic of the meeting will be nutritional support for chronically ill people.

At the end of January 2022, the group of patients looks as follows:

In February 2022 I visited our patients and their home carers in 5 communes at least once a month, getting to know their medical and non-medical needs. In meeting their needs, in cooperation with the network, I have taken the following actions:

1. I provided respite care for our patients’s home carers realized by a member of the Senior + Club in the village of Planta, Narewka commune.
2. I reported to the Secretary of the Commune Office in Narewka the need to urgently send a grader or other heavy equipment to unblock the access road to the colony in the village of Eliaszuki, as a result the road was leveled within a week.
3. The co-owner of the sewing room offered free coats, which I delivered to the patients’ homes.
4. I brought and helped to fill in an application to the County Family Assistance Center for co-financing from the funds of the State Fund for Rehabilitation of Disabled Persons. The application is for the elimination of architectural barriers (replacement of a bathtub with a door) in the commune of Zabłudów.

I participated in a recording for a cultural institution in Warsaw – John Paul II Thought Center and for the Podlaskie Voivodeship information portal – Podlasie Gate, where I talked about my role and activity in the project.

As part of educational activities the meetings with a home hospice nutritionist we held in five communities. The topic of the meetings was: “Nutritional support for chronically ill patients.”

– 02/03/2022 – Common Room in Lewków Stary (Narewka commune), members of the Senior + Club, Rural Housewives’ Clubs, employees of the Commune Social Welfare Center and caretakers of our hospice patients.

– 04/02/2022 – the Municipal Cultural Center in Zabłudów – members of Rural Housewives’ Clubs, school teachers, employees of the Municipal Social Welfare Center.

In the month of March 2022 I visited our pupils and their home carers 44 times and, in cooperation with the network, I took the following actions:

1. The respite care provided by a member of the Senior Club in the village of Planta in the Narewka commune is still ongoing.
2. I contacted the partners of the network regarding the possibility of hardening of the previously made driveway for cars in front of one of our patient’s house
3. I talked about my activities in the reportage project carried out by Polish TV Białystok
4. On the occasion of visiting charges, I conducted educational interviews on how to apply for home physiotherapy provided by the National Health Fund.
5. I had talks with hospice doctors about the qualification system for admission, continuation of treatment and discharge from home hospice.
6. There were meetings in 3 communes participating head of the commune, doctor and manager of the Communal Social Assistance Center, the subject of which were our activities in the project.
